To apply for our programs we recommend that you call our office to schedule a time to meet with one of our housing counselors to complete an application. If your schedule does not allow you to meet with one of our staff in person complete the application and drop off or mail it to our office at 213 South 1st Street, Norfolk, NE 68701. To help us determine your eligibility for assistance please also send copies of the following:
- Your most recent paystubs from adult wage-earners in the home
- Bank statements from all accounts
- Previous year tax return and W-2’s (2 years if self-employed)
- Social Security benefit documentation, if applicable
- Life insurance cash value records, if applicable
- Most recent statements from any retirement plans, stocks, mutual funds, or any investments that you have.
After we receive verification of your employment from your employer and the required documents listed above we will send you a letter letting you know if you are eligible for our assistance programs.
